Plan Effective Date Your coverage will begin on the latest of the following:
1. Your Arrival to USA; or

2. The date your Application and premium are received by Insurance company; or

3. The date your Application and premium are accepted by Insurance company; or

4. The date you request on the Application.
Plan End Date Your coverage will end on the earlier of the following:
1. The date shown on the Insurance Confirmation Card, for which premium has been paid; or

2. The date you return to your Home Country; or

3. 12 months after your original Effective Date; or

4. The day an insured becomes a U.S. citizen or is considered a U.S. resident by the state where they are residing; or

5. The date of entry into active military service.
