Its scheduled-benefit design helps keep premiums low while providing coverage for eligible medical expenses, helping protect travelers during their trip. Coverage is available from 5 days to 365 days.

While in the U.S., after the deductible is satisfied within the First Health PPO network, the insurance company will pay up to the stated amount in the policy document for each covered treatment or service.

ChoiceAmerica has a unique feature called US Border Entry Protection Benefit. This benefit is designed for foreign travelers entering the United States, helping mitigate the financial risk associated with being denied entry at a U.S. port of entry.

How does this plan pay?

ChoiceAmerica® is a fixed dollar based limited coverage plan. The plan pays a pre-defined fixed dollar amount for every service or treatment. These specific amounts are mentioned in the Plan Brochure and the Description of Coverage. The policy maximum is just the outer limit, but there are limits to every service or treatment.

Eligibility

  • Available for B1 and B2 visa holders residing outside of the U.S., while traveling to the United States, Mexico, or Canada
  • Coverage begins once the policyholder has left their home country and is in transit to or has entered one of the eligible destination countries
  • For ages 14 days to 79 years
  • Coverage for minimum of 5 days up to 365 days
  • Extendable up to 365 days
  • To qualify for coverage, you must not be a citizen permanent resident of the host country you are traveling to (U.S, Mexico, or Canada)

ChoiceAmerica provides travel medical insurance for non-U.S. citizens on a B1 or B2 visitor's visa traveling to the United States, Mexico, or Canada. To be eligible for coverage, you must not hold citizenship in the country you are traveling to: United States, Mexico, or Canada. ChoiceAmerica is not available to applicants who list a U.S. state address as their mailing address.

The ChoiceAmerica pre-existing rider is designed to include coverage for the worsening of their pre-existing conditions. The rider has specific limits as to what treatment and services are allowed and how much of it will be covered. Known treatment and services or on-going care pertaining to the existing condition(s) will not be covered with the optional rider. Pre-existing conditions are usually not covered in travel insurance policies. The ChoiceAmerica plan offers limited coverage for pre-existing conditions, only if you choose optional coverage for pre-existing conditions. This can be purchased with additional premium for a minimum of 30 days.
If you choose the optional pre-existing condition rider, after an additional deductible of $50 per incident, the plan will pay for the following:

1) Physician visit: $150 per visit, 3 visits maximum.
2) Lab & x-ray: $1,000.
3) Emergency room & hospitalization: $1,500.
4) Prescription: $100

Yes, Border Entry Protection coverage is a unique feature of the Choice America Plan. It is available for people traveling to the USA on a valid nonimmigrant visa. For some reason, if the entry to the USA is denied at immigration, the insurance would cover up to $550 for an economy class air fare, back to the insured person’s home country. The cost of ChoiceAmerica varies based on several qualifying criteria, such as the traveler's age, duration or length of trip, selected coverage benefit limits, and the selection of additional riders.

Your ChoiceAmerica policy may be extended in increments with a minimum of five days up to the maximum coverage period of 12 continuous months. There is a $5 administration fee for extensions.

Yes, you can cancel the ChoiceAmerica plan. If you submit a written request to International Medical Group (IMG) prior to your start date, you will receive a complete refund. After the policy's start date, you may submit a written request for cancellation as long as no claims have been submitted, but the following conditions will apply:
1) You will be required to pay a $20 cancellation fee and
2) Your refund will be pro-rated based on the amount of days left on the policy. If you have filed claims, your premium is non-refundable.

You cannot remove one person from the ChoiceAmerica policy if other travelers are included. We recommend letting the current policy lapse with all travelers or canceling and repurchasing for the remaining travelers as per the policy terms.
In order to apply for ChoiceAmerica, you will need full name(s), coverage duration, date(s) of birth, home country address, and a valid credit card.

ChoiceAmerica plan is an online insurance policy. Once you purchase the policy online you will receive a printable policy in your e-mail. You can print and cut the paper ID card for your record and use it in the event you go to Doctor or hospital.

ChoiceAmerica's Influenza (flu) Vaccination benefit will allow an insured person the ability to get a one-time flu vaccination up to $50. The benefit is only available in the United States and has a $25 co-pay. Treatment and services are subject to the terms and conditions of the certificate. ChoiceAmerica will provide coverage in all eligible countries, except your Home Country and any restricted countries listed in the policy. Coverage is valid only within the policy dates. Note: If any part of your trip includes the United States, please select it as your destination country.

ChoiceAmerica covers eligible COVID-19 treatment and related medical services if the illness is contracted after the policy effective date. However, COVID-19 vaccines, boosters, and other preventive care services are not covered. COVID-19 testing is covered only when medically necessary, ordered by an attending physician for diagnostic purposes due to symptoms; tests required for travel are excluded. The plan also does not include a quarantine benefit, so please refer to your policy documents for complete details, limitations, and exclusions.

Coverage will begin on the date selected and once the policyholder has left their home country. The ChoiceAmerica policy will only cover for conditions that occur after the policy effective date unless the optional Pre-Existing condition rider is purchased.
Only the primary destination will be listed on the ChoiceAmerica policy documents, but all eligible countries will still be covered during the policy period.
The ChoiceAmerica plan starts coverage at 12:01 am EST on the chosen start date and once the traveler leaves their home country. Coverage applies worldwide outside the home country as long as the policy is active and the destination country is not restricted. Coverage ends immediately upon re-entry into the home country or at 11:59 pm EST on the last coverage date, whichever comes first.

The deductible is the initial amount the insured must pay before the insurance covers costs. For the ChoiceAmerica Policy, you can choose a deductible of $0, $50, or $100. For limited coverage plans like ChoiceAmerica, it's better to choose the lowest deductible because the plan pays fixed amounts for services. A higher deductible further reduces the insurance payout

  • For example, with a $50,000 policy and a $50 deductible, if the plan pays $65 for a doctor's visit, the $50 deductible applies, so the plan pays ($65 - $50) = $15.
  • If you return for another visit, the plan pays $65.
  • For the ChoiceAmerica Plan, the deductible is applied once per policy period.
  • No, the ChoiceAmerica plan does not extend or renew automatically. You will need to sign into your account online and manually activate the extension or renewal before the policy expires. Not all plans are extendable or renewable, so be sure to review your policy terms and conditions for eligibility.

    The ChoiceAmerica plan does not offer any coverage for quarantine.

    The insurance provider recommends submitting correction requests in writing, as your policy is a legally enforceable document. Once received, we forward the request to the provider, who will process requests in the order they are received. It typically take 3-5 business days, for the correction to be completed. To avoid errors and corrections, please double-check your ChoiceAmerica application before purchasing.
    The ChoiceAmerica pre-existing rider does not cover for any cancer related treatment and services. The policy explicitly excludes charges incurred for radiation therapy or Treatment and chemotherapy.

    Dental is usually not covered in travel insurance policies. However, the ChoiceAmerica plan covers up to $700 for offers a limited coverage for dental accidents. There is also additional coverage for dental and vision if you add the optional coverage coverage.     1. Up to $100 for one dental exam, and

    Pre-certification is the process of letting the insurance company know, before taking any major medical services like Hospitalization, surgery etc. In case of an emergency admission, the precertification call must be made within 48 hours of admission, or as soon as reasonably possible. If a hospital admission, a surgery or anything else requiring precertification is not pre-certified, eligible claims and expenses will be reduced by 50%.     The benefits and cost does not differ if you buy individually or combined ChoiceAmerica policies. If the visitors have different travel plans (might be coming together but leaving on different dates, etc.) it is recommended to buy separate policies. Having separate policies gives you more flexibility in renewals and cancellations.
