How the Benefits are Paid
After the Insured pays the deductible:
Inside PPO Network: The plan pays 90% of eligible expenses up to US$5,000, then 100% up to the Policy Maximum.
Outside the PPO Network: The plan pays 80% of eligible expenses up to US$5,000, then 100% up to the Policy Maximum.

Home Country Definition
U.S. Citizens: Home Country is always the United States of America.
Non-U.S. Citizens: Home Country is the country of which the Insured Person is a citizen or national; including any country where the Insured Person maintains his/her primary residence or usual place of abode and any country of which the Insured Person is the possessor of a validly issued passport. In the event there is more than one home country under the above-listed criteria, the Home Country is the country meeting the above-listed criteria and listed by the Insured as his or her Home Country on the application.
US States Restrictions
Residents of Maryland (MD) state cannot purchase this policy. Applicant with Maryland (MD) state address cannot purchase this policy. Policy still provides coverage for policy holder who is visiting MD State

Q1: What information do I need to purchase this plan?
Name(s), date of birth, and passport number of the visitor(s).
Q2: When will I receive my ID card?
You can print out your ID card from your confirmation e-mail and it will be used as proof of insurance. While filling out the online application, if you select online fulfillment you will not receive anything in the mail (paper mail), but you will receive confirmation in your e-mail. If you select paper mail/snail mail then you will receive the ID card in the mail within 5-10 business days. You can also retrieve your ID card using MyIMG: Get My ID Card
Q3. What's the Rx Discount Card that comes with this plan?
This is a discount savings program available to every certificate holder of the Patriot travel plans. This program allows card members to purchase prescriptions at one of over 35,000 participating pharmacies in the U.S. and receive the lower of 1) Universal Rx contract price or 2) the pharmacy regular retail price.
This discount program is not insurance coverage. It is purely a discount program to purchasers of the Patriot travel plans. Use of the discount card does not guarantee that prescribed medication is covered under the insurance benefit plan.

Q4. What's the Return Rider? Should I get it?
Patriot America Insurance automatically covers the flight from home country to the U.S. but if you want the trip back to home country covered as well then you should purchase the Return Rider. This rider is an additional cost and you'll be able to get a quote for it and select it in the application.

Riders are additional features or additional coverage to the standard plan. Not everyone needs all the riders. Patriot America Insurance has the following riders to choose from. All the riders have an additional cost and you'll be able to get a quote for it and select it in the application.

The PPO provider for Patriot America Insurance is FirstHealth PPO Network. FirstHealth PPO Network is an independent organization and a recognized PPO Network by all major hospitals, doctors, and health facilities.
You can search for doctors or hospitalsin this Provider Directory. Prefer going to any doctor or hospital listed under this PPO network to receive in-network benefits benefits.

Q9: When I buy this plan, do I need to make the full payment at once or do I have the option of paying it on a monthly basis?
Based on the desired coverage length you have selected, you have to make the full payment at once.
Since this plan is renewable you can renew it anytime you would like to before the policy expires. There is a renewable fee of $5 that associated with the renewal.
