A

For the ChoiceAmerica Plan, the coverage for Urgent Care would be the same as that of a doctor’s visit. Depending on the policy maximum you choose, for covered doctor’s or urgent care visits, after the deductible is met, the benefit amount is $65 for $50,000 policy maximum or $90 for $100,000 policy maximum. Doctors Visit/ Urgent Care visits are allowed 10 times in a policy period.

If you choose the Optional Pre-existing condition rider, and need to visit an Urgent care facility for that, then after the deductible is met, the plan will pay up to $100 for each visit. Maximum two visits are allowed for pre-existing conditions per policy period.
