My dad had kidney disease and underwent kidney transplant an year ago. He is also diabetic. He has been on transplant medication and insulin ever since. He is stable and takes his medication as prescribed and has experienced no problems since the transplant. He is planning to visit the USA. If he gets into any related problems while in USA, will the "acute onset of pre-existing conditions" cover the medical costs? Who decides whether the condition would qualify for this coverage? Appreciate your help on this.

Acute onset refers to an unexpected emergency situation that could be fatal for the patient. Whether or not, this procedure qualifies as an acute onset of pre-existing conditions is determined by the attending physician. However, it would not cover for medication, check-ups, etc.
