There are two ways to file a claim with your insurance provider, depending on how the medical services were billed:

Direct Billing by the Provider: If your provider billed the insurance company directly and proof of insurance was provided at the time of service, you typically don’t need to file a claim. However, some providers may still require you to submit a Claim Authorization Form, so it’s a good idea to check with them to confirm their requirements.

Out-of-Pocket Payment:If you paid for medical expenses or services upfront and want to file a claim for reimbursement, you’ll need to provide medical records, bills, and a completed claim form to your plan administrator.
