Patriot Platinum insurance is a travel medical insurance plan that provides medical insurance coverage and emergency services to individuals and families traveling outside their home country. After the deductible is satisfied, for treatments within the PPO network, the plan pays 100% up to the policy maximum. Outside the PPO network, the plan pays 90% for the first $5,000 and 100% thereafter. Outside the USA, the plan pays 100% after deductible.

Eligibility
Can be purchased by an non-U.S. citizen traveling outside of their home country on a legal visa status. Non-United States citizens or any visa holders (e.g. L/H/B1, B2, etc.) as well as person on Green Card living outside of the U.S. can also purchase the plan for temporary coverage in the U.S. For someone below 65 years of age, coverage must begin within 6 months of arrival in the U.S. and for someone 65 years of age and older, coverage must begin within 30 days of arrival in the U.S. (restriction waived with proof of previous valid insurance within 30 days of the policy start date).

Home Country Definition
U.S. Citizens: Home Country is always the United States of America.
Non-U.S. Citizens: Home Country is the country of which the Insured Person is a citizen or national; including any country where the Insured Person maintains his/her primary residence or usual place of abode and any country of which the Insured Person is the possessor of a validly issued passport. In the event there is more than one home country under the above-listed criteria, the Home Country is the country meeting the above-listed criteria and listed by the Insured as his or her Home Country on the application.

Residents of Maryland (MD) state cannot purchase this policy. Applicant with Maryland (MD) state address cannot purchase this policy. Policy still provides coverage for policy holder who is visiting MD State

Q1: Am I eligible for the Patriot Platinum Plan? The Patriot Platinum Plan is available for any non-US resident, visiting the USA or anywhere outside their home country, for a short period of time. | ||||||
Q2.What Information do I need to buy the Plan? You would need the names, dates of birth, passport numbers of the visitors, and a valid credit card to buy the plan. | ||||||
Q3: When will I receive my ID card? You can print out your ID card from your confirmation e-mail and it will be used as proof of insurance. While filling out the online application, you will receive confirmation in your e-mail. If you select paper mail/snail mail then you will receive the ID card in the mail within 5-10 business days. Q4.Can I extend the plan, if I plan to stay longer? Yes, if you buy the plan initially for more than 30 days, you can extended the policy. You would need to extend the plan at least two days before the policy expires. There would be an extra $5 extension charge. | ||||||
Q5.Can I cancel the plan, if I plan to go back earlier? Yes, you can cancel and get a refund for the Patriot America Plan, if you need to go back earlier than plan. The following rules apply: | ||||||
Q6. Are pre-existing conditions covered by the Patriot Platinum Plan? Pre-existing conditions are not covered by the Patriot Platinum Plan. However, the Patriot Platinum Plan covers for an acute onset of a pre-existing condition. It covers, with varying limits by age, up to $1,000,000 for people up to 69 years of age; For age 70 and over, the plan does not provide coverage for acute onset of a pre-existing condition. It is up to the attending physician to determine if a particular medical situation is related to pre-existing condition or not, and also if it would be considered as an acute onset or not. Please check the Description of Coverage for more information about what is considered as acute onset of pre-existing condition. | ||||||
Q7. What is an acute onset of a pre-existing condition? An acute onset is a sudden and unexpected outbreak or recurrence of a Pre-existing Condition, which occurs spontaneously and without advance warning either in the form of Physician recommendations or symptoms, and is of short duration. It has to be an acute medical condition that requires immediate medical attention. More conditions and restrictions apply. Q8.What's the Rx Discount Card that comes with this plan? This is a discount savings program available to every certificate holder of the Patriot travel plans. This program allows card members to purchase prescriptions at one of over 35,000 participating pharmacies in the U.S. and receive the lower of 1) Universal Rx contract price or 2) the pharmacy regular retail price. This discount program is not insurance coverage. It is purely a discount program to purchasers of the Patriot travel plans. Q9. What is the Sports riders options? Adventure Sports are usually not covered in travel medical policies. With additional premium you can purchase the optional Adventure Sports Rider Coverage. It is available for those up to the age of 64. The following activities are covered to the lifetime maximum amounts listed below as long as they are engaged solely for leisure, recreation, or entertainment purposes: abseiling, BMX, bobsledding, bungee jumping, canyoning, caving, hang gliding, heli-skiing, high diving, hot air ballooning, inline skating, jet skiing, jungle zip lining, kayaking, mountain biking, parachuting, paragliding, parascending, piloting a non-commercial aircraft, rappelling, rock climbing or mountaineering (ropes and guides to 4500m from ground level), scuba diving (to 50m), skydiving, snorkeling, snow boarding, snowmobiling, snow skiing, spelunking, surfing, trekking, whitewater rafting (to Class V), and wildlife safaris, and windsurfing. All such activities must be carried out in strict accordance with the rules, regulations and guidelines of the applicable Governing Body or Authority of each such activity.
