Q1: Who is Eligible for the International Major Medical Insurance?

  • Non-US citizens between 14 days and 84 years old traveling to the USA

Q2: Which doctor or hospital can I go to? You can go to any doctor or hospital of your choice. If you would like, you can choose to go in the PPO network for International Major Medical, which is the FirstHealth PPO Network.
You can search for doctors or hospitals in the Provider Directory.

Q3: How will I receive my ID card?
The ID card along with the policy certificate will be mailed to you by traditional US postal Services after you been approved of the coverage.

Q4: How long can I purchase this insurance for?
You can purchase this insurance for a maximum period of 180 days. You can repurchase the plan subject to underwriters approval

Q5: How do I buy this insurance?
You have two options to purchase this insurance. Either online or by paper application. processing fee of $100 is charged if you choose to apply through paper application. The coverage is subject to approval by the underwriters. Time taken for approval varies from instant approval to 4-5 business days.

Q6: How do I use this insurance?
After purchasing the policy online, you will receive your ID card in your confirmation email. You can go to any doctor/hospital of your choice within the specific geographic area. The doctor/hospital can either ask you to pay up front or will bill the insurance company directly. You will need to go through a mandatory claim process afterwards. Benefits are subject to the terms and conditions as outlined in the certificate.

Q7: How do I file a claim?
Written notice of a claim must be submitted to the insurance co. within 20 days after the eligible expense incurred. On receiving the notice of claim, the underwriters will furnish the claim form.
The claim from must be submitted within 90 days after the date eligible expense are incurred along with the following documents.

  • Completed Claim form.
  • Signed authorization for release of medical records.
  • Original receipts form providers, or copies of cancelled checks or credit card payments plus a copy of a provider statement or bill.

All receipts must contain legible information to determine the name and address the Provider; the diagnosis; the treatment rendered; the date of service; and the payment made to Provider. In addition, Underwriters reserve the right to verify Your proof of loss by obtaining any or all necessary medical records or other necessary information from other sources. This will be obtained at Underwriter's expense.

Q8: When I buy this plan, do I need to make the full payment at once or do I have the option of paying it on a monthly basis?
This policy has a monthly payment plan. The application will ask for your credit card information. The monthly payment will be automatically deducted from the corresponding credit card account.

Q9: I want to purchase insurance for more than one visitor. Should I purchase two separate policies for them or have a combined policy?
You would need to buy the Plan separately for everyone. You cannot buy a combined policy for spouses or children. The pricing is based on the age of the insured & number of days chosen.
