After the deductible, your responsibility is 10% for up to $5,000, which is $500. After this, insurance will pay for covered expenses up to the policy limit. The amount is applicable for all of your bills, within your policy period. You do not have to pay each time. Bascially, your maximum responsibility is 1. In-Network: Deductible + $500 ( i.e. 10% of first $5,000) 2. Out-of-network: Deductible + $1,000 ( i.e. 20% of first $5,000). Coverage amounts and out of pocket responsibility vary by plan, so be sure to read your policy details carefully before purchasing.
