How does payment for medical treatment work?

The billing process is typically up to the providers themselves, and payment usually happens in one of two ways: direct billing or upfront payment.

Direct Billing: In this case, the provider bills the insurance company directly. This is more common for larger treatments, like hospitalizations or surgeries.

Upfront Payment: For smaller services, like doctor consultations, the provider may ask you to pay upfront. You can then file a claim with your insurance to be reimbursed for the covered amount.

Limited plans, like ChoiceAmerica pay a fixed dollar amount for every service and treatment. The policy maximum is simply the outer limit for coverage, and any deductible is deducted from the fixed benefit amount, not the provider’s charge.
